Easy Referral
Please complete the form below or fax the completed PDF to 512‑938‑3991. NPI information and Provider Name noted in form.
For comprehensive care, please include the patient’s medical history, most recent lab results, and any current concerns.
Seamless Intake
Once we receive the referral, our team will reach out to your patient within 24–48 business hours to schedule their nutrition counseling session.
We share updates and coordinate with your team to support integrated patient care. A link for an additional release of information (ROI) form is below for other non-referring providers.

Alejandra Amezola-Velazquez, MS, RD, LD, is a bilingual, Spanish-speaking registered dietitian based in Austin, Texas, specializing in plant-forward nutrition, women’s health, hormone balance, blood sugar support, and metabolism. She works collaboratively with healthcare providers to support patient care and coordinate nutrition guidance.

Amezola Nutrition does not treat diagnosed eating disorders such as anorexia nervosa, bulimia nervosa, or ARFID. However, we specialize in supporting adults with disordered eating behaviors, including chronic dieting, restrictive eating, emotional or boredom-related eating, overeating, and unhealthy relationships with food or body image. These behaviors are common in today’s “diet culture” and can benefit from safe, individualized nutrition guidance and strategies to build a healthier relationship with food.

Amezola Nutrition specializes in women’s health, but we work with clients of all genders. We provide personalized nutrition guidance for adults, supporting a variety of goals including overall wellness, digestive health, hormone health, and sustainable lifestyle changes.

Yes! Our practice is 100% virtual. Virtual appointments are available and conducted through a secure, HIPAA-compliant platform called Practice Better.

Initial appointments typically last 75 minutes, and follow-up appointments are usually 55 minutes, depending on your needs.

Your first session is dedicated to getting to know you and building a strong foundation for your nutrition plan. The provider will take a deep dive into your:
Medical history – past and current conditions, lab results, and health concerns
Medications and supplements – what you’re taking and how it may impact nutrition
Nutrition habits – current eating patterns, preferences, and challenges
Lifestyle factors – activity level, sleep, stress, and routines
Health goals – short-term and long-term objectives
This comprehensive assessment allows us to create a personalized, realistic plan that is tailored to your needs and informs all follow-up sessions, ensuring your plan is effective, sustainable, and aligned with your goals.

To get the most out of your first session, we recommend having the following ready:
Recent lab results or medical procedure records
A list of current medications and supplements
Any other relevant health information
All documents and information can be uploaded in advance through the Practice Better client portal. Your provider and you will review these together during the session to ensure a thorough understanding and create a personalized plan.

A Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) is a credentialed healthcare professional. They now must complete a master’s degree, a supervised practice program, pass the Commission on Dietetic Registration exam, and maintain state licensure. Both RD and RDN are protected titles, meaning only qualified professionals can use them.
A nutritionist, by contrast, is not a regulated title. Anyone can call themselves a nutritionist, and training programs vary widely in quality and oversight.
RDs and RDNs are trained to provide medical nutrition therapy and can help prevent or manage conditions such as diabetes, gastrointestinal disorders, eating disorders, and other health concerns—offering evidence-based guidance you can trust.
